The incredibly unlikely confluence of events causing the meltdown is Alex, right? I think we are SUPPOSED to think it seems like a wildly unlikely coincidence.

In the process of meltdown I think there were smaller explosions and rumbles before the big boom causing the tiles to fall.
If I recall correctly the plan for the engineer guy to escape was to start a meltdown which then would trigger the o2 alert and force the door open. Due to him getting blown up the process was not stopped as intended.

No I think the show set it up as "here's a weird combination of events that would never happen naturally, and it's a known issue with the system but so unlikely it's not worth addressing." And my take is that Alex, through...whatever mechanism it uses...set events in motion so that both things would happen simultaneously.
